This flowchart is here to help navigate the licensure process. If you're stuck in your licensing process, this questionnaire will lead you through the steps and forms you'll need to complete.

Step One:

Fill out Form 1, then fill out either Form 2 (sent to your Student Services Coordinator) or Form 20 (your school files Form 20 without you)

Step two:

After you are approved from Albany, you will receive a link to register with the ASWB. The ASWB will verify your eligibility. This will take up to 7 days.

Step Three:

Register at Pearsonvue Testing Center
‍Once you are approved by the ASWB, you will be able to register a date to take your exam with Pearsonvue testing centers. There are dates every day. You can cancel your date as many times within a year as you want without penalty. You can cancel your test date up to 24h before your exam.

After you have completed 2000 client contact hours, 36-72 months of consecutive or non-consecutive employment at approved sites within a consecutive 72-month span, 100 supervision hours.

Step One (Albany) :

Form 1 (completed online)
Form 2 (sent to your student services coordinator)
Form 4 (sent to Albany)
Form 4B (sent to your supervisor/s)

Once Albany receives all forms, it will take 6 weeks to review and approve. If it has taken longer than 6 weeks. Call them.

Step One (Connecticut Department of Public Health) :

Submit the following forms through the portal:
Application Form
School Transcript
‍Employment Form
‍Supervision Form
‍Application Fee is $200

Step two:

After you are approved from the CT Department of Health, you will receive a link to register with the ASWB. The ASWB will verify your eligibility. This will take up to 7 days.

Step Three:

Register at Pearsonvue Testing Center
